History
Subject, an autonomous body registered under Charitable Trust Act was established
on 27th July 1998 by Municipal Corporation of Greater Mumbai (MCGM) for control
and prevention of HIV/AIDS in Mumbai. In NACP II from 1998 to 2005 STI
component had strengthened 18 STI/RTI clinics (STD and OBGY dept) at tertiary (

In accordance with
National AIDS Control Organization (NACO) – World Bank mandate, AIDS Control
Societies were established in all the states and selected corporation areas
during the period 1998-1999. These societies were established with the
objective of implementation of National AIDS Control Programme (NACP) Phase II
within their respective jurisdiction. As a result, 38 autonomous State AIDS
Control Societies were established all over the country with adequate financial
and administrative powers to identify and respond to local needs.
Accordingly Mumbai Districts AIDS Control Society (MDACS), an autonomous body
registered under Charitable Trust Act was established on 27th July 1998 by
Municipal Corporation of Greater Mumbai (MCGM) for prevention and control of
HIV/AIDS in Mumbai.
MDACS is serving as a nodal agency employing multi-sectoral approach collaborating
with concerned stakeholders viz. General Health Sector, NGOs, CBOs, People
Living with HIV/AIDS, Corporate Houses and Media etc. The strategy employed is
decentralization, making the use of existing infrastructure, provision of
technical and financial support, promoting sharing of experiences and expertise
among all the partners.
MDACS
In a nutshell, MDACS mission can be summed up as a 4-pronged strategy:
Prevent the spread of AIDS
Reduce the vulnerability of individuals and
communities to HIV/AIDS
Alleviate the socio-economic and the human impact of
the epidemic
Provide care and support to those infected and
affected by the disease.

About HIV / AIDS
HIV (Human Immunodeficiency Virus) is a virus. someone is HIV infected, or has HIV infection. These are terms that mean that the HIV was detected in their blood. If adequate care is not taken than they can pass the virus to other people.
HIV attacks the body's immune system. The immune system protects the body from
infections and disease, but has no clear way to protect itself from HIV.Without
treatment, most people infected with HIV are less able to fight off germs that
tthy are exposed to everyday., Many of these germs do not usually make a
healthy person sick,but they can cause life-threatening infections and
cancers.in a person whose immune system has been weakened by HIV. Treatment for
HIV can slow this process and allow people with HIV to live longer, healthier
lives
People infected with HIV may have no symptoms for ten or more years. They may
not know they are infected. An HIV test (blood test discussed below) is the
only way to find out if you have HIV.
AIDS stands for Acquired Immune Deficiency Syndrome. It is a late stage of HIV
infection
A person with HIV infection has AIDS when he or she:
Has a CD4 cell count (a way to measure the strength of the immune system) that falls below 200. A normal CD4 cell count is 500 or higher. OR
Develops any of the specific serious conditions - also called AIDS-defining illnesses - that are linked with HIV infection
* Following activity
are conducted in the 23 designated STI/RTI clinic in year 2008-09 and in
24 designated STI RTI clinic in year 2009-10 as per NACO guidelines.
Designated STI/RTI Clinics to control Sexually Transmitted Infections (STI)
Sexually transmitted infections are these infection which are transmitted from one person to other by having unsafe sexually contact with infected persons i.e. unsafe sexual contact with multiple partners. In presence of STI the transmission of HIV increases 5 to 10 times. Early, effective and complete treatment of STI prevent the transmission of HIV by 50%. So as a part of National AIDS Control Programme, Syndromic approach for diagnosis and management of Sexually Transmitted Infections (STIs) is adopted by National AIDS Control Organized (NACO) Roll of subject.
Subject has developed 24 STI/RTI clinics in Municipal and Government Hospitals
in Mumbai. These were further strengthened by subject as per the National
guidelines put forwards by NACO.
This strengthening is done in the following ways
Funds are provided for consumables to diagnose the STI/RTI as per National guidelines, for daily OPD of Skin – STD and OBGY dept. of Public Health setup.
Capacity building of Medical and paramedical staff is done for management of STI/RTI patients as per National guidelines.
Free supply of STI/RTI colour coded drug kits for treatment of STI/RTI patients as per NACO norms.
Counseling services are provided at 24 designated STI/RTI clinics by appointment of trained STI counselors at these centers. The services of –
A] STI/RTI counseling, D] Partner notification and Management,
B] Follow up treatment, E] Condom promotion and
C] Risk reduction behavior education are given to the patients by these STI counselors.
Reporting of STI/ RTI patients treated in designated STI /RTI to subject CMIS every month.
Periodic supportive Supervisory Monitoring of 24 designated STI/RTI clinics services is done by the Medical college faculty Members by visiting these Hospitals every 2 months for improving the STI/RTI services as per NACO norms .
The reports of quality monitoring of 24 designated STI/RTI clinics are submitted to NACO periodically.
STI services in Targeted Intervention (TI) component:
There are 27 STI/RTI clinics run by subject for a) FSW, b)
MSM, c) TG and d) IV drug user. Technical and financial support
are provided through TI component at NGO sites. These STI
clinics of TI , NGO/CBO provide STI services to high risk group free of
cost in the form of :-
Counseling Treatment and
Diagnosis Condoms promotion and risk reduction
Referral linkages of TI NGOs are established with designated STI /RTI clinics
in public sector for advanced services like etiological diagnosis of STI and
HIV along with the treatment of STIs and referral to the ART
centers of HIV positive patients.
There are 36 TI projects one each of 1000 head count are provided funding for
strengthening the TI STI clinic of the respective NGO /CBO from TI component.
Part time STI doctor is appointed in each of these TI STI clinics from TI
component. The training of these doctors is conducted for syndromic
case management.
These TI STI clinics do monthly reporting of STI /RTI episode treated in them
to subject CMIS.
STI/RTI services in Public Private Partnership:
Population Services International (PSI) is identified agency by NACO for Public
Private Partnership in Mumbai city and Mumbai Suburban. The following co
ordination is done by subject with PSI for providing STI/RTI services in
public private partnership.
MOU with the100 private practitioners identified by PSI in each district i.e. a) Mumbai City b) Mumbai Suburban is made by subject through PSI.
Technical Support is given to PSI for training of these private practitioners.
Provision of STI/RTI drugs kits as per NACO norms are distributed to them through PSI for social marketing.
Periodic Supportive Monitoring visits of these private practitioners are conducted by medical college faculty members as per NACO norms to improve the STI/RTI services given by them.
Reporting of STI/ RTI patients treated by Private practitioners is done to subject CMIS every month through PSI.
Implementation of Youth Friendly Health Services for the adolescent
of age group 15 to 24 years in G.S. Medical College and K.E.M Hospital
Skin and STD OPD from Monday to Friday between 2.00 to 3.00
pm as per National Training Programme of Adolescent Reproductive and
Sexual Health (ARSH ) by November 2009-10 on pilot basis.
Establishing the linkages between this “Youth Friendly Clinic” (tertiary
level) to the ARSH clinic at Sub-district level under NUHM.

AIDS SCARE
This is what they call a nightmare repeating itself. Over 74,000 people, who got themselves tested for HIV infection at BMC-run hospitals and health centres between February '07 and May '07, must undergo the test all over again. Reason-the test kits used at these centres during this period have been found to be faulty.
The Mumbai District AIDS Control Society (MDACS) has issued a notice in two newspapers — one English and the other Marathi-admitting that those who underwent HIV tests at BMC centres in February possibly went home with wrong results.
While the notice is as cold and dispassionate as a government notice can possibly be, imagine the chaos it is going to cause in the lives of those it is directed at. A person who may have been declared HIV positive and who may have pulled himself out of all social contact because of the stigma attached to an HIV+ve, will now wonder whether he wasted all these years.
At the same time, somebody who went home happy after being declared HIV-ve will now wonder whether he has infected his partner and/or whether his children too are at risk now.
Though MDACS refused to share any information about complaints, a Goregaon
resident recently found that the HIV test results at the civic-run M W Desai
Hospital had contradicted the results of a subsequent test at a private medical
hospital in Malad. The mother of two suffered a miscarriage allegedly because
of the mental trauma she underwent after she was found HIV+ve following the
test at the BMC hospital.
Vaijayantee Bagawe, member of Committed Communities
Development Trust, an NGO working for AIDS and HIV patients in
Bagawe has a point. Forget about helping these people, the BMC is unlikely to be able to even contact them because HIV tests are conducted under complete confidentiality.
“It is not mandatory for a person to mention his real name and address when
undergoing an HIV test. He or she can simply mention Mr and Ms and write the
registration number. So we do not have addresses to contact them. We therefore
appeal through the media that they should take the test again,” said S S
Kudalkar, deputy municipal commissioner and head of MDACS.
THE GOOF-UP
HIV tests on 74,764 people were conducted between February ‘07 and May ‘07 at BMC-run hospitals and health centres under the aegis of MDACS.
HIV detection kits from batch number 9180, manufactured by Span Diagnostics,
were used during this period. These kits were later found to be unreliable. The
Delhi High Court subsequently asked the National AIDS Control Society to
conduct fresh tests.
“There was a writ petition in the HC in 2007 after several people questioned
the results and it was revealed that the kits used for testing could be faulty.
The court then passed the order last year. Our own reference library that looks
after quality control also detected that the kits were defective. We found that
there were disparities between two results for the same person,” said Kudalkar.
The BMC used 1,660 HIV detection kits during the said period. “In July 2007,
NACO asked us to stop using kits manufactured by M/s Span Diagnostics. Now we
have been asked to purchase kits from four registered companies. We have also
been asked to conduct fresh tests and submit a report to the court. As the
matter is subjudice, I cannot comment further,” Kudalkar added.
The Mumbai Districts AIDS Control Society (MDACS) has issued a notice stating
that the results emerging from HIV/AIDs blood tests conducted in 2007 in 64
BMC-run hospitals could be faulty.
